• Medical Information: GP details, medication and relevant medical history.

We collect your personal information via email and over the phone.

We use your information for the following purposes:

• Booking our assessment: To arrange our initial assessment via phone.
• Communication: To send booking confirmations, updates, and important information regarding any appointments.
• Safeguarding: To ensure that we are able to act appropriately in the case of emergency.

We do not share personal information with any other party, with the exception of cases where we are required by law to break confidentiality for safeguarding purposes. In this case we will discuss this with you before doing so, but may share only essential information with relevant authorities or emergency services.

If you book an assessment, but do not become a client, we will delete your personal information within twenty-eight (28) days. We are required by law to retain client information for a period of seven (7) years.
